Information for Patients
Patients who have changed their name, address, telephone number, e-mail address, or insurance carrier are requested to notify our office. It is imperative that we have current and correct information on all of our patients. We provide “auxiliary aids and services” for our patients who need such services.
Office Hours
Office hours are by appointment only. We schedule appointments from 8:00 AM until 4:30 PM weekdays.
If you have a dental emergency after regular office hours, a telephone answering machine will take your call and tell you where Dr. Kham can be reached. This service is EXCLUSIVELY for our patients of record. We suggest that new patients call during regular office hours.
When you call our office, please notify us of the nature of your problem or concern and if you feel an immediate appointment is necessary. If it is an emergency, we will do our best to work you into our schedule the same day. Please call as early in the day as possible so that you can be seen promptly.
It is best to call for elective appointments 1 or 2 weeks in advance so that a convenient time can be arranged.
Appointments and Cancellations
We understand that our patients’ time is valuable, and we always respect your time. Except for emergency treatment for another patient, you can expect us to be prompt. We would appreciate the same courtesy from you.
When your appointment is made, we are exclusively reserving our time and preparing a treatment room for your particular needs. We ask that if you must change an appointment, please give us at least 24 hours (1 business day) notice. This courtesy makes it possible to provide your reserved time and room to another patient who would like it. Your appointment is considered confirmed when it is made or requested by you. We will call or send a reminder text as a courtesy to remind you of the appointment date and time.
Cancellations within 24 hours are subject to cancellation fees. Missed appointments or patients who fail to show up for scheduled appointments without giving required notice are subject to the same cancellation fees. Repeated cancellations or missed appointments may result in the loss of future appointment privileges.
We understand that unforeseen situations can arise on your way to the office, and for this reason, we will be flexible with occasional lateness. However, please be aware that arrival after your appointment time may require your appointment to be rescheduled.
Informed Consent/Refusal of Treatments
You will always be given detailed verbal information regarding the dental procedures you will receive. Post-operative instruction (if any) will also be given verbally. The risks, benefits, alternatives treatment options, and consequences of no treatment will be explained at length. You will be given the opportunity to ask any question regarding your treatment options, and they will be answered to your satisfaction before we start any treatment.
Verbal consent is the usual way our patients provide consent. Written explanations and consent forms are available upon request and also available as downloadable PDF files from our practice website.
